Long-Term Patency of PTFE Artificial Vascular Grafts in Peripheral Bypass

Peripheral arterial disease (PAD) is a common chronic vascular disorder that seriously endangers limb blood supply and even leads to limb amputation in severe cases. Peripheral vascular bypass surgery is the main clinical treatment for severe peripheral arterial occlusion, which uses vascular grafts to establish alternative blood circulation pathways and restore limb blood perfusion. Autologous great saphenous vein has always been the gold standard for peripheral bypass grafts, but it is limited by insufficient autologous vascular resources, venous lesions, and poor patient physical conditions. PTFE artificial vascular grafts have become the most important alternative material for peripheral bypass surgery due to their abundant sources, stable mechanical properties, and good surgical adaptability. The long-term patency rate of PTFE grafts directly determines the long-term clinical efficacy of peripheral bypass surgery and is the core index to evaluate graft application value PTFE SHEET.

Long-term patency of peripheral bypass grafts refers to the ability of vascular grafts to maintain unobstructed blood flow and normal vascular morphology for a long time after implantation, avoiding stenosis, occlusion, and graft failure. Clinical data shows that the long-term patency rate of PTFE peripheral bypass grafts is affected by multiple factors, including graft material performance, surgical anastomosis technology, patient blood lipid and blood sugar levels, postoperative anticoagulation treatment, and local hemodynamic environment. Compared with autologous veins, traditional PTFE grafts have slightly lower long-term patency rate in small-diameter peripheral bypass, but with the continuous upgrading of material modification and structural optimization technology, the patency gap has been significantly narrowed, and modified high-performance PTFE grafts have even achieved comparable long-term efficacy with autologous veins in medium and large-diameter peripheral vascular reconstruction.

The excellent structural stability of PTFE materials is the fundamental guarantee for long-term patency of peripheral bypass grafts. Peripheral blood vessels are located in limbs, with frequent limb movement, repeated vascular stretching, compression and bending, which put forward extremely high requirements on the fatigue resistance and structural stability of grafts. High-quality PTFE vascular grafts prepared from high-purity PTFE TUBE raw materials have uniform micro-porous structure, excellent flexibility and bending fatigue resistance, and will not produce structural distortion, wall collapse and lumen stenosis after long-term repeated bending and compression. Different from polyurethane grafts that are prone to aging and cracking, PTFE grafts maintain stable structural morphology and mechanical properties for more than 10 years in vivo, effectively avoiding mechanical occlusion caused by structural failure.

Hemodynamic adaptability optimization is the key to improve the long-term patency rate of PTFE peripheral bypass grafts. Unreasonable graft compliance mismatch and anastomotic stenosis are important causes of early and medium-term graft occlusion. Modern optimized PTFE vascular grafts adjust the wall thickness and micro-porous structure gradient, realize elastic compliance matching with natural peripheral blood vessels, reduce vascular wall shear stress difference at the anastomosis, and avoid intimal hyperplasia caused by long-term hemodynamic turbulence. At the same time, the ultra-smooth inner wall of modified PTFE grafts reduces blood flow resistance, avoids local blood stasis and platelet aggregation, and effectively reduces the incidence of anastomotic thrombosis and restenosis.

Biological modification technology further improves the long-term biological stability of PTFE peripheral bypass grafts. Heparin anticoagulant modification, endothelialization modification and anti-inflammatory modification can effectively inhibit thrombosis, intimal hyperplasia and chronic inflammatory stimulation after graft implantation. For elderly patients, diabetic patients and hyperlipidemia patients with high risk of vascular stenosis, modified PTFE grafts can significantly improve long-term patency and reduce the risk of secondary surgery. Clinical follow-up data shows that the 5-year patency rate of modified PTFE peripheral bypass grafts can reach more than 75%, and the 10-year patency rate remains above 60%, which is far higher than that of other synthetic vascular materials.

In conclusion, with the continuous progress of material optimization and clinical application technology, PTFE artificial vascular grafts have become a reliable long-term bypass material for peripheral arterial disease. By optimizing mechanical stability, hemodynamic adaptability and biological compatibility, the long-term patency performance of PTFE grafts continues to improve, providing stable and effective surgical solutions for patients who cannot use autologous blood vessels, and reducing the amputation rate and reoperation rate of peripheral vascular disease.
